Case Management is an all-encompassing process that helps to bring the catastrophically injured client from their current state to a state of individualized maximum community integration. Case Management may include: the creation of a community rehabilitation team following hospital discharge, the coordination of medical and specialist appointments, acting as a liaison between the client, the family, the medical community and the rehabilitation team, coordinating each step towards returning to school and/or independent living, exploring vocational placements and reintegration into the community.
